How to book an appointment

We now offer a total triage system, this system assess patients based on their needs and directs them to the most appropriate care.

To request an appointment with a doctor, either routine or urgent on the day, you must complete an online consultation form, which can be found here. Appointments can no longer be booked directly by phone. However, the care navigator team can help complete the form by phone or at the desk for those without internet or smartphone access.

The online consultation form is available Monday to Friday 08:00am – 06:30pm, excluding bank holidays and staff training closure afternoons. It can be completed online or through the NHS App under the services section.

Nursing appointments can still be booked directly with reception.
