Travel Advice

Foreign travel

You may need some vaccinations before you travel; you can complete and submit the travel form online (below) or pop into the Surgery to collect one if you prefer to complete a paper version (to complete and return to reception).

Once you have submitted your forms, please contact Reception to book an initial assessment with a Practice Nurse to discuss what immunisations or other advice/treatment you may require.

We ask that all travel forms are submitted 8 weeks before you travel, in order that you can plan your vaccines accordingly.

Many vaccines need to be given at least 2 weeks before you travel and  some courses of vaccines will take longer so they need to organised well  in advance.

The Practice Nurse will give you a travel leaflet which will contain information and contacts you may need to book an appointment with a specialist travel clinic in the case that you require non-NHS vaccinations.
