Long Term Conditions – reviews

Long-term conditions (chronic diseases) can require regular reviews to ensure they are monitored and managed appropriately.

Patients who are on our registers for the following conditions will be invited for periodic Long Term Condition Reviews  –  Heart Disease, Heart Failure, Stroke and TIA, Diabetes, Hypertension, COPD, Asthma, Mental Health, Kidney Disease, Dementia, PAD, Rheumatoid Arthritis, Non-Diabetic Hyperglycaemia, Atrial Fibrillation and Learning Difficulties.

To make it easy for you to remember, we now invite patients in the month of their birth.  You may receive your invite to book an appointment by either text or letter.

Most Long Term Condition Reviews are conducted by the nursing team.  The reception team will book your appointment with the most appropriate member of the team.
