Welcome and thank you for considering becoming a patient at Summervale Surgery!

If you would like to register to become a patient at Summervale Surgery you can register online via the link below;

Or print and complete the appropriate form using the links below.  Please return completed forms to our Reception desk.

Alternatively, printed forms can be picked up from Reception for you to complete.

You must complete one form per person.

Please note that, at present, the registration process can not be completed on-line.

New Patient Registration Form – Adult (18 years of age and over)

New Patient Registration Form – Child (under 18 years of age)

For each new patient we also require a completed Family Doctor Registration Form – GMS1. This form is available at our Reception Desk.

We can only accept patients who reside within our practice area.  The practice boundary map is shown below for you to check.
